Insurance
Aspen Midwifery is contracted as in-network with the following insurance companies:
Aetna
Blue Cross Blue Shield
Cigna
Unfortunately, we cannot accept AHCCCS, Medicare, Tricare, or United
Because each company offers many different plans, we strongly encourage every patient to contact their insurance company directly to verify whether Aspen Midwifery is in-network for their specific plan and how their benefits apply. You are responsible for any co-pay, co-insurance, or remaining deductible at the time of your appointment
Self-pay
We offer discounted rates of 20% off for patients who do not have insurance or who prefer to pay out-of-pocket. To qualify, payment must be made in full at the time of service
As a general guide, new patient visits are typically $150-300 and follow up visits are typically $100-200
Patients with out-of-network insurance plans or healthshare memberships are considered self-pay. Upon request, we will gladly provide billing documentation for you to submit to your plan for possible reimbursement
Homebirth Package
Together with our biller, we can provide you with a personalized estimate for pregnancy and homebirth care with us. This estimate is based on information from your insurance company, or the self pay estimates, and our biller can review these details with you directly
Understanding Your Coverage
Insurance can be complex, and no two plans are alike. While we wish we could simplify how coverage decisions are made, these processes are determined by each insurance company and are entirely outside of our control. For the most accurate information about how your plan will apply, we encourage you to contact your insurance company directly before your visits. This ensures you know what to expect from them and allows us to focus fully on providing excellent care

Certain elective services are not billable to insurance
Please inquire for details about these offerings, including pricing
In such cases, payment is due at the time of your visit
